Exercise-induced mitral regurgitation and right ventricle to pulmonary circulation uncoupling across the heart failure phenotypes

چکیده
Ex-MR is a common mechanism across the spectrum of HF subtypes and combines with ventilatory inefficiency RV-PC uncoupling. Interestingly, in HFpEF, emerged as unexpectedly prevalent peculiarly associated increased chronotropic response peripheral O 2 extraction potential adaptive mechanisms to backward flow redistribution.

Right Ventricular Contractile Reserve and Pulmonary Circulation Uncoupling During Exercise Challenge in Heart Failure: Pathophysiology and Clinical Phenotypes.
OBJECTIVES Right ventricular (RV) exercise contractile reserve (RVECR), its phenotypes, and its functional correlates are among the unresolved issues with regard to the role of the right ventricle in heart failure (HF) syndrome, and understanding these issues constitutes the objective of this study. BACKGROUND Although the role of the right ventricle in HF syndrome might be fundamental, the p...
متن کاملErratum to: The right ventricle: interaction with the pulmonary circulation
The primary role of the right ventricle (RV) is to deliver all the blood it receives per beat into the pulmonary circulation without causing right atrial pressure to rise. To the extent that it also does not impede left ventricular (LV) filling, cardiac output responsiveness to increased metabolic demand is optimized. Since cardiac output is a function of metabolic demand of the body, during st...
متن کاملSecondary Mitral Regurgitation in€Heart€Failure
Ma The development of secondary mitral regurgitation (MR) due to left ventricular dysfunction, also known as functional MR, is strongly associated with a poor prognosis in patients with heart failure. The mechanisms underlying secondary MR are multifactorial; accurate imaging assessment of secondary MR may be challenging and nuanced; and the appropriate roles of medical, surgical, and intervent...
متن کاملFeasibility and efficacy of bypassing the right ventricle and pulmonary circulation to treat right ventricular failure: an experimental study
BACKGROUND Right ventricular failure (RVF) and -support is associated with poor results. We aimed for a new approach of right - sided assistance bypassing the right ventricle and pulmonary circulation in order to better decompress the right ventricle and optimize left ventricular filling. METHODS From a microaxial pump (Abiomed), a low resistance oxygenator (Maquet and Novalung) and two cannu...
متن کاملThe right ventricle and pulmonary circulation: basic concepts.
The primary purpose of the right ventricle and pulmonary circulation is gas exchange. Because gas exchange occurs in thin, highly permeable alveolar membranes, pulmonary pressure must remain low to avoid pulmonary edema; because the right ventricle and the lungs are in series with the left ventricle and the systemic circulation, the entire cardiac output must pass through the lungs. This low pr...
